DOL Updates FMLA Forms
The DOL (Department of Labor) has published new FMLA (Family and Medical Leave Act) forms that covered employers may use to provide required notices to employees and employees can use to provide certification of their need for leave for an FMLA-qualifying reason. Use of the forms is optional, but any other forms used by the employer must provide and request the same basic information.
The DOL revised the forms to make them easier to understand and use. The forms are electronically fillable PDFs, now have more questions with check-box responses, and include electronic signature features. The updated forms and a FAQ can be found HERE.
